Multidisciplinary team approach to diabetes. An outlook on providers’ and patients’ perspectives

Objective/BackgroundThis study sought to uncover the perspectives of various stakeholders towards multidisciplinary team (MDT) care, discover new understandings and help inform current practice on MDT care for diabetic patients.Methods5 electronic databases were searched for articles that evaluated patients’ and providers’ perspectives on type 2 Diabetes Mellitus (T2DM) MDT management. Articles retrieved were sieved, coded and findings were analytically themed together in accordance to Thomas and Harden methodology.Results15 articles were identified with three common themes: interactions between healthcare providers, benefits to patients and constraints and facilitators of the healthcare system. Trust and synergistic teamwork are important factors in promoting effective care. Patients commended MDT's improved accessibility and convenience and felt more welcomed. Often plagued by poor support, lack of manpower and resources, MDTs are less efficient and incapable of realizing their full potential.ConclusionThis review illustrates that the MDT model does improve diabetes treatment outcome, help prevent or reduce complications. Nevertheless, the MDT model can be a double-edged sword as poor interactions between HCPs can hamper quality patient care. The current MDT model is also based on available resources of the health system. More effort is needed to modify the MDT model to meet the changing needs of patients.

浙江省德清县县域医共体改革措施与效果分析

目的：分析浙江省德清县县域医共体改革措施及阶段性成效。方法：定性访谈了解医共体整合措施，定量分析医共体建设前后（2016-2018年）各级医疗机构医疗服务利用数据及医共体部分绩效考核数据。结果：医共体在组织、管理、功能、服务方面进行整合，并取得阶段成效：医保基金在县域内支出率有所增加；乡镇卫生院卫生人员数量、医疗收入有所提高，乡镇卫生院住院病人人均医药费有所降低。结论：医共体取得初期成效，应开展以患者健康结局指标为主的长期追踪调查，全面系统评估医共体；保留基层医疗机构地位、功能定位不变；探索实行医共体"总额预付、结余奖励"制度，将医共体发展成以患者健康为中心、基于利益共享机制、分工合作的责任利益共同体。     

Engaging the Health Care Professional

There is no shortage of discussion surrounding the importance of achieving a highly engaged workforce in health care institutions. Despite the abundance of idealisms surrounding the importance of providing patient-centered care, some medical radiation technologists may consider this to be a contradiction, when they themselves do not feel valued or empowered by their employer. In addition, overwhelming evidence points to the fact that high employee engagement is directly linked to positive patient experiences and outcomes. Logically, it then follows that much attention should be paid to fully engaging health care teams. Highly engaged medical radiation technologists are more likely to provide personalized patient-centered care. Health care leaders can help to engage their teams by using effective behavioral and leadership styles that will be received favourably by most staff. With the evolution of the generations occupying the workplace, managers must adapt and vary their leadership styles, if they hope to effectively engage their teams and provide world-class patient-centered care.
